Terms of Service

The ground rules for using Carebridge Collective’s website, resource connections, donations, memberships, and provider directory.

These terms explain what visitors, donors, members, providers, and Carebridge can expect when using the website and its services.

1. Acceptance and eligibility

These Terms of Service (“Terms”) govern your use of the Carebridge Collective (“Carebridge,” “we,” “our,” or “us”) website and related online forms, directory, donations, memberships, and communications. By using the website or completing an online transaction, you agree to these Terms and the Privacy Policy.

You must be at least 18 years old, or have the permission of a parent or legal guardian, to make a donation, purchase a membership, submit an organization listing, or otherwise enter into an agreement through the website.

2. Information and connection—not medical care

Carebridge provides educational information, community resources, navigation support, and opportunities to connect with independent providers and organizations. Website content is for general informational purposes and is not medical advice, diagnosis, treatment, a prescription, a care plan, or a substitute for advice from a qualified professional.

Carebridge does not guarantee that information is complete, current, or appropriate for a particular person. Always confirm services, credentials, insurance, eligibility, accessibility, telehealth availability, and other important details directly with the relevant provider or organization.

3. Emergencies and urgent needs

Carebridge does not provide emergency services and does not monitor website forms continuously. Do not use the website for an emergency, urgent medical issue, or immediate safety concern. In the United States, call 911 for an emergency or contact an appropriate crisis or healthcare resource.

4. Provider directory and third-party services

Directory listings are provided to improve visibility and access to information. A listing, membership, link, or event participation does not constitute Carebridge’s endorsement, certification, referral guarantee, or warranty of credentials, suitability, availability, quality, or outcomes.

Providers and organizations are independent from Carebridge unless expressly stated otherwise. They are responsible for their own services, statements, credentials, privacy practices, and legal compliance. Any relationship or agreement you enter with a third party is between you and that third party.

Members who submit a listing represent that their information is accurate, current, lawful, and authorized for publication. Carebridge may format, correct, decline, suspend, or remove a listing to protect users, address inaccuracies, maintain directory standards, or comply with law.

5. Memberships and annual billing

Carebridge memberships are offered at the level and annual price displayed during signup. Unless the checkout clearly states otherwise, an annual membership is a recurring subscription that renews automatically each year using the payment method on file until canceled.

You authorize Carebridge and its payment provider to charge the displayed amount, including any approved additional Corporate Champion amount, at signup and at each renewal. Carebridge will provide the price and billing frequency before payment is completed.

You may request cancellation before the next renewal through the Carebridge contact page. Cancellation stops future renewal and does not automatically reverse a completed charge or end directory participation before the close of the paid term. Any refund or adjustment is subject to the written terms presented at checkout, applicable law, and Carebridge’s approved policy at the time of the request.

Carebridge may pause or end membership benefits or a directory listing for nonpayment, misuse, inaccurate information, risk to the community, or a material violation of these Terms.

6. Donations and recurring gifts

Donations are voluntary charitable contributions and do not purchase professional verification, endorsement, governance authority, care priority, or preferred directory placement. Donors may choose a one-time gift or, when available, a monthly recurring gift.

By selecting monthly billing, you authorize the displayed amount to be charged each month until canceled. You may request cancellation of future charges through the Carebridge contact page. Completed donations are generally final except where a refund is required by law or approved by Carebridge because of an error or exceptional circumstance.

Carebridge may provide a donation receipt. You are responsible for determining the tax treatment of a contribution and should consult your own tax advisor.

7. Communications consent

Carebridge may send service-related email or text messages about a donation, membership, directory listing, event, request, or account. If you separately consent to promotional communications, Carebridge may also send news, opportunities, campaigns, and similar updates.

Consent to marketing text messages is not a condition of making a donation or purchasing a membership. Message frequency varies and message and data rates may apply. Reply STOP to an eligible text message to opt out and HELP for assistance. Carriers are not liable for delayed or undelivered messages.

8. Your content and submissions

You retain ownership of content you submit. You grant Carebridge a nonexclusive, worldwide, royalty-free license to store, reproduce, format, display, and use submitted content as reasonably necessary to provide the requested service, maintain an authorized public listing, communicate about Carebridge programs, and administer the website.

You represent that you have the right to submit the content and that it does not violate another person’s rights or applicable law. Do not submit patient information, medical records, confidential third-party information, or content you are not authorized to share.

9. Acceptable use

You may not use the website to:

  • violate law, another person’s rights, or these Terms;
  • misrepresent identity, credentials, services, affiliation, or authority;
  • submit harmful, deceptive, discriminatory, defamatory, or infringing content;
  • introduce malicious code, interfere with security, or attempt unauthorized access;
  • harvest information, send spam, or use automated systems in a manner that burdens the website; or
  • copy or exploit Carebridge content or branding except as permitted by law or written authorization.

10. Disclaimers and limitation of liability

The website and its content are provided on an “as is” and “as available” basis to the fullest extent permitted by law. Carebridge does not warrant uninterrupted access, error-free content, a particular provider match, referral, result, recovery outcome, or availability of any third-party service.

To the fullest extent permitted by law, Carebridge and its directors, officers, employees, volunteers, and agents will not be liable for indirect, incidental, special, consequential, exemplary, or punitive damages arising from website use, third-party services, directory information, or reliance on general website content.

Nothing in these Terms excludes liability that cannot legally be excluded. Your rights may vary by jurisdiction.

11. General terms, changes, and contact

Carebridge owns or licenses the website design, text, graphics, logos, and other content, except for member content and third-party materials. These Terms are governed by the laws of the State of Arizona, without regard to conflict-of-law rules, except where another law must apply.

If a provision is unenforceable, the remaining provisions continue in effect. Failure to enforce a provision is not a waiver. These Terms, the Privacy Policy, and transaction-specific terms shown at checkout form the applicable agreement for website use.

Carebridge may update these Terms as services or legal requirements change. Continued use after an updated effective date means the revised Terms apply to future use.
